Residential Property Owners
- Print and sign a copy of the petition using your legal name. You must include the MAP with the petition, either on the back or attached on a separate page.
- Have more than one property owner? Have both owners sign and return the petition.
- Is your property held in a Trust? Print, sign and date the Trust form for your residential property.
- Mail your completed petition to:

- Sign for all the businesses you may own.
- Select the PDF form above for the type of business you own (Corporation, LLC or Partnership)
- Fill out the form carefully.
- Print the business address and sign, including your authority to sign the petition and form (i.e. President, Manager, Partner)
- Mail your completed petition to Anna Clare Shepherd at the address above.

- If you have more than one property, either residential or business, please sign for them all on the same page and send them in together.
